Fractional Director of Operations & Business Development (Home Care)
At Home by Silver Horizons is a licensed non-medical home care agency seeking an experienced home care professional to work directly with the Owner and CEO to support caregiver recruitment, business development, client acquisition, operations, and agency growth.
This is a hands-on leadership role for someone who understands how home care agencies operate and has experience generating referrals, building relationships, growing service hours, and helping agencies increase client census.
PRIMARY OBJECTIVE
Help At Home by Silver Horizons continue its growth by recruiting caregivers, generating client opportunities, developing referral relationships, increasing service hours, and supporting agency growth and operational readiness.
KEY RESPONSIBILITIES
• Recruit, screen, and onboard caregivers
• Develop and maintain referral relationships
• Support business development and agency growth initiatives
• Identify and pursue opportunities to increase client census and service hours
• Conduct outreach to discharge planners, case managers, physicians, senior living communities, rehabilitation facilities, adult day centers, fiduciaries, and other referral sources
• Represent the agency at networking and community events as needed
• Assist with scheduling, staffing, payroll, and agency operations
• Improve operational workflows and processes
• May lead caregiver orientation, onboarding, and training sessions as needed
• Work directly with the Owner to achieve agency growth goals
REQUIRED QUALIFICATIONS

• Minimum 3 years of experience in a licensed non-medical home care agency
• Knowledge of Illinois Department of Public Health (IDPH) Home Services Agency requirements
• Must reside in Lake County, Northern Cook County, or a neighboring community and be available for occasional in-person meetings
• Must have reliable transportation and be willing to travel locally for referral development, networking, caregiver onboarding, training, and agency-related activities
• Previous experience as an Agency Administrator, Director of Operations, Branch Manager, Operations Manager, Community Liaison, Business Development Representative, Staffing Manager, Scheduler, Recruiter, or similar home care leadership role
• Experience recruiting and onboarding caregivers
• Experience with scheduling, staffing, payroll, and agency operations
• Demonstrated experience developing referral relationships, generating client referrals, increasing census, growing service hours, or contributing directly to business development within a home care agency
• Ability to work independently and execute without extensive supervision
PREFERRED QUALIFICATIONS
• Existing relationships with discharge planners, case managers, physicians, senior living communities, rehabilitation facilities, social workers, fiduciaries, or other referral sources in Lake County, the North Shore, or surrounding communities
• Experience helping grow a startup or early-stage home care agency
• Experience conducting caregiver orientation and training
• Experience with CareSmartz360, AxisCare, AlayaCare, WellSky, HHA Exchange, or similar software
COMPENSATION
$50–$65 per hour based on experience + BONUS
(Performance bonus opportunities available based on client acquisition, caregiver recruitment, service hour growth, referral development, and agency growth milestones.)

Pay: $50.00 - $65.00 per hour
Expected hours: 10.0 – 15.0 per week
